An exciting opportunity has arisen for an Assistant Practitioner to join the new and transformational One Coventry Integrated Team (OCIT) service. The trust along with Coventry City Council and Coventry and Warwickshire Partnership Trust have designed and developed 3 Local Integrated Teams (LITs) across the city of Coventry which are supported by multi-professional teams offering a holistic and integrated approach to urgent community needs for the residents of Coventry. To work as an Assistant Practitioner within One Coventry Integrated Team as a member of a multidisciplinary team The post holder will provide rehabilitation to adults in Coventry with a wide range of conditions under the direction of a team of qualified therapists, in patients current residence, or other community setting (e.g. short term placement). Assist and lead in supporting individualised treatment programmes assessing and reviewing outcome measures and supporting effective service development. In addition to these responsibilities following training and completion of clinical competency document the Assistant Practitioner will be able to deliver delegated therapy assessments and interventions with a level of autonomy equal to their qualification and competency. As required, to work flexibly to ensure appropriate service provision over a 7 day working pattern. To adhere to the lone worker policy. There will be an on-going and daily requirement for the post-holder to travel to different locations within Coventry and Warwickshire The post holder will be required to have a full driving licence and car and to be insured for business use. To have knowledge and understanding of a wide range of conditions to be able to contribute to and undertake therapy assessment and treatment To deliver delegated therapeutic assessment or treatment according to the patients’ goals and complexity, with co-workers or as a lone worker, according to established competency. To regularly visit patients in their own homes and other community settings. To assist in moving and handling and therapeutic positioning of patients to provide assessment and intervention. This may include assisting the patient getting up and down off the floor, managing stairs and working in confined spaces. To progress an individual’s therapy programme autonomously, seeking professional guidance and supervision when required if scope of practice is exceeded. Using equipment, prescribed for the individual within therapeutic tasks or sessions. Ordering and assessing for basic therapy equipment with delegated patients following completion of assessed competency. To demonstrate knowledge and skills in the use and application of specific equipment to enable the training of others (including patients, carers, relatives and staff) to a given competency. To work collaboratively with Social Services and third sector providers to ensure delivery of effective safe care.
