The Old School House Care Home is seeking a Team Leader to join our Care Team. We are a dementia friendly home, specialising in residential care and support for elderly. We are looking for a compassionate and caring person to join us as a Care Team Leader.
Job Title: Care Team Leader (Full time and/or Part Time considered)
Weekly Hours: From Part time - 24 hours per week, to full time hours.
Location: The Old School House Care Home, Plympton, Plymouth, PL7 1TL
Job Summary:
As a Care Team Leader you are responsible for leading and managing the daily care shifts in the home. You will be responsible for ensuring whole team delivery of compassionate and personalised care to individuals who require assistance with their daily living activities in a residential care home setting. Your role is vital in supporting individuals to maintain their independence, enhance their quality of life, and promote their overall well-being. Responsibilities
* Ensure all care plans are reviewed accurately to reflect the service users changing needs
* Maintain regular contact with relatives and other health professionals
* Take responsibility for allocations and the smooth running of the shift
* Ensure accountability and completion of Team Leading tasks
* Review and request prescriptions
* Identify and refer to health professional when appropriate
* Order, sign in and sign out drugs as per medication policy
* Administration of medication, and check/re-check for missing signatures
* Take accountability for the delivering, administering, and signing of all prescribed medications
* Ensure you are familiar and carry out emergency procedures in a prompt manner
* Ensure regular observations are completed on the care floor to ensure the staff team work effectively
* Check flag status of all service users on a regular basis, following up any outstanding tasks with the appropriate team member
Monthly Duties
* Review care plans and complete individual senior tasks
* Carry out care staff supervisions as requested
Day to Day Duties
* Receive handover and deploy staff accordingly
* Delegate tasks to team accordingly to competence/training
* Tour house and observe/communicate with each service user
* Monitor personal care requirements and presentation of service users
* Supervise serving of meals and monitor dietary and hydration requirements
* Check diary for days events
* Ensure telephone is answered in a timely manner
* Undertake the administration of medicines
* Work as part of a multidisciplinary team communicating with other professionals to ensure service users needs are met i.e. GP, DN etc
* Show perspective service users and families around the home when required
* Respond to visitors requests and demands, offering the appropriate hospitality
* Monitor team performance and practice
* Respond to staff feedback and problem areas
* Monitor daily reports by care staff and take appropriate action where required
* Monitor cleaning standards and odour control
* Review/amend care plans and update risk assessments
* Ensure communication books are updates and handover to next shift
Qualifications and Skills:
· Education: A level 2 in health and social care is preferable or a high school diploma or equivalent qualification, but not essential. Additional certifications in health and social care or related fields are advantageous.
· Experience: Prior experience as a Care Assistant and Senior carer in a similar caregiving role is preferred.
· Empathy and Compassion: Possess a caring and empathetic nature, with the ability to connect with individuals of diverse backgrounds and varying physical and cognitive abilities.
· Communication Skills: Excellent verbal and written communication skills, including active listening and the ability to communicate effectively with clients, their families, and the care team.
· Patience and Adaptability: Demonstrated patience, tolerance, and adaptability to meet the unique needs and preferences of individual clients.
· Physical Stamina: Good physical health and stamina to handle the demands of the job, including lifting, assisting with transfers, and being on your feet for extended periods.
· Reliability and Dependability: Exhibit reliability, punctuality, and the ability to manage time effectively, ensuring all assigned tasks are completed in a timely manner.
· Integrity: Maintain a high level of professionalism, ethical conduct, and respect for confidentiality.
· Problem-Solving Skills: Display the ability to assess situations, identify issues, and apply appropriate solutions while adhering to established protocols and guidelines.
· Flexibility: Willingness to work flexible hours, including weekends, evenings, and overnight shifts, as required by the organisation or the individual clients' needs.
