Who You Are You are a motivated and compassionate individual with a Social Care qualification, such as NVQ/QCF Level 3 in Adult Social Care or equivalent. With at least 2 years of experience supporting vulnerable adults to achieve reablement and maximize their independence, you are adept at carrying out medication needs assessments and risk assessments to implement personalized care plans for carers to follow. You possess excellent communication skills in English, both verbal and written, and are able to converse with customers courteously and provide accurate advice. You're comfortable using IT systems including Excel, Word, and internal service systems to maintain electronic records. A full clean driving license and access to a vehicle are required to travel between service user homes and office premises. What the Job Involves The role of a Reablement Officer is centered on providing comprehensive support to service users to help them plan, work toward, and achieve their care and health outcomes while maximizing their independence through reablement. Responsibilities include working closely with service users to assess their needs, design, plan and implement care plans, and support case management as directed by the line manager. You'll assist in maintaining effective working relationships with service users, offering practical support to meet their needs both individually and as a team. Partaking in professional development through training and workshops is essential, as is collaborating with various stakeholders, including health professionals, voluntary organizations, and other council departments. Compliance with council policies, statutory requirements, and data protection regulations is critical, as well as participating in health and safety practices. You will work a flexible schedule within a 4-week rolling rota, with placements available for 28 or 35 hours per week, including occasional weekend work.
