The Hospital Discharge Team is a highly professional, supportive, caring and dedicated Adult Social Care Team working across all areas of Central Bedfordshire operating across several Hospitals.
The service works in close partnership with other health and care systems, and we work alongside our Community Health Service provider to deliver a multi-disciplinary approach to Integrated Health & Social Care.
We are currently recruiting to our team and are looking for aSenior Practitionerto join us. We maximise people’s independence by practicing in a person centred in a strength-based manner with a strong focus on improving people’s futures.

This post is key in supporting and developing the day-to-day operation of the team and supporting the team manager in overseeing the continuous approach we have in improving and developing our practice. Working alongside your peers, providing support in the Hospitals, ensuring the timely and proportionate allocation of work within the team, providing supervision, support, and guidance to staff.
About you
You will have significant Health and/or Social Care experience, including multi-disciplinary and inter agency working. Your ability to transfer your skills will help us achieve our goal in the delivery of excellent person-centred approaches. You will value the impact of sound evidence-based practice and understand that best practice results in best value. You will know the importance of enabling people to be supported to be as independent as possible for as long as possible and that having a strong connection with their local communities can make a real difference to people's lives and overall wellbeing. As a Senior Practitioner you will need to demonstrate an excellent value base to the people we support and also your colleagues.
Expected main roles and responsibilities:
1. Carry a reduced, but complex caseload, if required
2. Provide monthly supervision to registered and non-registered practitioners and annual appraisal/development discussion.
3. Providing day-to-day support, advice and guidance to direct reports and other staff members where required.
4. Oversight of the Duty function- provide guidance, manage queries, and support escalation.
5. On call rotation in weekends
6. Quality Assurance for any direct reports
7. Support Plan validation.
8. Audits
9. Allocations
10. Facilitation of Peer Reflective Groups.
11. Completion of RTW discussions and RTW forms with direct reports
12. Interviewing as part of the recruitment process
13. Attendance at various meetings
You must have a recognised social work qualification and be currently registered with Social Work England. You will also have a minimum of three years post qualifying practice and experience. Your values will demonstrate excellent social work skills in practice and professional pride.
