Digital health Virtual Ward Assistant Practitioner/RNA Band 4 hours per week
This is an exciting post within Digital Health, this is designed to support the Virtual Ward Team.
The successful candidate will work within the Digital Health Team supporting patients to remain at home using remote monitoring, and video assessments.
These patients are supported from GP Ward round and a Consultant ward round daily which you will provide to support to.

Main duties of the job
The Assistant Practitioner will work as part of the Digital Health Team, be multi-skilled and will work across various disciplines within Digital Health and organisations including SFT. They will provide planned care and be able to take appropriate action to achieve the planned aims. The AssistantPractitioner will be proficient, competent and require minimum supervision. The role requires the Assistant Practitioner technical / clinical skills of the post will be further enhanced as part of an induction.
