This is a 31.1 hrs per week post which will enable the right candidate the opportunity to be part of a busy Reablement and Care at Home. The Nairn & Ardersier Reablement team, provide an important and valued service focusing on assessments at home swift discharges to the residents of the Nairn & Ardersier area. The post will assist with assessing and utilising an enablement approach identifying outcome plans to individuals and their families/carers. The focus will be on maximising independence and supporting the individual to remain in their own home, facilitating prompt discharge from hospital and supporting anticipatory care planning to prevent readmission or admission to a hospital. Requirement to be registered with the Scottish Social Service Council within 3 months of appointment.
