Job summary An exciting opportunity has arisen for an appropriately skilled and experienced registered nurse/AHP to join Kirklees Health and Care Partnership's Continuing Healthcare (CHC Team) as a case manager for the physical disability CHC Team. Although direct experience of working in a CHC team is not essential, candidates must be able to demonstrate knowledge and experience of the CHC legislation and process and have experience of working within the physical disability field of practice. The post holder will be responsible for holding a portfolio of complex cases including Physical Disability and Neuro- rehabilitation with a specific view to ensuring that current care packages are responsive to individual needs as assessed. All reviews will be carried out in line with the requirement of both the National Framework for NHS Continuing Healthcare and funded-Nursing Care (revised July 2022) and the relevant commissioning principles guidance. The CHC Care Manager will work under the guidance and support of the Continuing Care Clinical Leads with responsibility to the Head of CHC.
