We are currently seeking health and social care assistants with a broad range of previous experience and knowledge who are looking for an opportunity to develop skills in rapid response and management of acutely unwell clients in the community. You will be working under the guidance of multi-disciplinary clinicians, to support assessment and treatment of patients to achieve medical stability and maintain functional ability, aiming to prevent unnecessary hospital admissions.
Main duties of the job
You should have previous experience of working with a therapy, nursing or personal care team within a health or social care setting. You must be flexible, responsive and have excellent time management skills as the team's priorities can change throughout the day. You should have good written and verbal communication skills and enjoy working in a dynamic and effective team. As the service is community-based, you will need your own means of transport and will be required to work long days (7am to 7pm) and weekends on a rota basis.
