This is a 30-hour-per-week role.

Main duties of the job
The post holder will work as a member of the Practice Team. The post holder will be responsible for recognising, negotiating and implementing processes to enhance the delivery of care.
Patient-centred care will be delivered by assessing patients with undifferentiated and undiagnosed problems; initiating treatment, investigations, advice and management of problems using professional autonomous decision-making processes, liaising when appropriate with other healthcare professionals and service providers to ensure safe and effective patient care.
The post holder will contribute to the development and delivery of a high-quality innovative service to the community through the implementation of evidence-based clinical practice, supported by continual professional development and mentorship/supervision.

Job responsibilities

1. Communication
1. Communicates effectively as a member of a multi-disciplinary team.
2. Refers when necessary to general practitioners, Paramedics, Accident and Emergency, Secondary Care, District nurses, Community MTD, social workers, mental health team, and Social Care Teams including third sector organisations.
3. With Primary care nurses giving support, advice and leadership.
4. With reception staff regarding patient information.
5. Communicates effectively and respectfully with patients and their family and carers assisting them to navigate the care pathway.
6. Recognises and effectively manages situations where there are barriers to effective communication.
2. Clinical Skills
1. Works independently as an Advanced Clinical Practitioner within the scope of professional practice assessing both adults and children.
2. Functions at a high level to carry out clinical decision making initiating investigation, treatment, advice and management of undetected and undiagnosed problems.
3. Practices as a non-medical prescriber.
4. Demonstrates clinical competence in the assessment and management of patients of all ages from infants to the elderly and management of lifelong illness such as diabetes, asthma and chronic obstructive pulmonary disease during the time of acute illness and managing minor illness.
5. Delivers safe, effective evidence-based care and advice to ease patient problems.
Person Specification
Experience
* Significant post-registration experience (minimum 3 years in relevant field acute or primary care setting)
* Evidence of autonomous practice assessing & managing patients with undifferentiated presentations
* Evidence of good practice
* Leadership Skills
* Experience of working within primary or urgent care
Qualifications
* Current NMC
* Independent Non Medical Prescriber V300
* Evidence of recent Continual Professional Development
* ILS Intermediate Life support
* MSc Advanced Practice
* 998 or equivalent teaching certificate.
Other requirements
* Willingness to undertake further training and to teach others
* Current driving licence and access to own vehicle
* Research interest
* Experience of innovative methods of delivering care
Skills and Aptitudes
* Demonstrate clinical knowledge in acute and chronic disease management
* Demonstrate clinical competence in minor illness management
* Able to demonstrate excellent communication and consultation skills
* Able to work independently and effectively within a team
* Flexible and motivated
* Basic computer skills.
