Job summary Calderdale Memory Assessment Service are seeking to recruit a Band 5 nurse to work within their team on a fixed-term 8 month contract. This exciting opportunity is to work 37.5 hours per week with the Memory Assessment Service. The post is based at The Laura Mitchell Centre in Halifax. You would be working with a well-established multidisciplinary team that supports staff to develop both personally and professionally. We have a mix of professionals within the team and also access to Dietetics and Psychology. A full comprehensive induction will be provided and access to further training options. Calderdale Memory Assessment Service has recently received their MSNAP re-accreditation, which is endorsed by the CQC. The service has also received a Sustainable Mental Health Service Commendation. You will need to be able to work autonomously as well as within a team and be able to assess risks for people with dementia. Main duties of the job The postholder will work within the remit of the Band 5 job description which involves: Undertaking initial assessments of service users Intervention work, risk management and clinical decision making in line with leadership. Work in a collaborative manner with others to ensure seamless service delivery. Participate in the use of reflective practice, nursing research, and clinical supervision. The service places staff clinical and management supervision, appraisal, support and professional development as a high priority. Experience of working within the community would be advantageous.
