Support adults with complex needs, working 2-3 days per week in Bexley (Civic Offices) and engaging with the local community. Comprehensive assessments, personalised care plans, multi-agency collaboration, and person-centred care are key.
Responsibilities:
* Conduct assessments of individuals' needs in line with the Care Act 2014.
* Develop and implement person-centred care plans, ensuring statutory compliance under the Care Act 2014.
* Collaborate with multiple agencies to ensure integrated support, following the Mental Capacity Act 2005 and Care Act 2014.
* Participate in Court of Protection work, including attending hearings if required, in accordance with the Mental Capacity Act 2005.
* ID and address safeguarding concerns, ensuring protection under the Care Act 2014 and Mental Health Act 1983.
Requirements:
* Relevant social work qualification
* 3-4 years post-qualification experience and supervision experience
* Knowledge of Court of Protection work and attendance when required
