P2R Community substance misuse service in Dunstable hub have a fixed term 12 month contract to cover Maternity leave.

• To manage a caseload of 35-70 service users, depending on complexity and other responsibilities
• To carry out drug and alcohol misuse assessment including assessment of personal strengths and recovery capital, and risks.
• To develop recovery plans using a person centred approach to develop, implement and evaluate needs and strengths based care plans and risk management plans in partnership with service users, carers and other professionals.
• To offer health promotion and deliver harm reduction interventions, including needle exchange and access to physical health care including BBV and health screening

• To liaise with professionals e.g. GP at start of treatment, at review and discharge
• To provide evidence based drug and alcohol interventions in line with guidelines e.g. NICE, CQC, in a variety of settings promoting flexibility and choice for service users.
• To deliver prescribing interventions, working in partnership with medical colleagues; titration, monitor and review maintenance regimes, detoxification etc.
• To maximise engagement with service users and reduce the risks both to the individual service user and to others affected by their drug and/or alcohol misuse.
• To facilitate service users and carers to be involved in care and treatment.
• To refer service users to appropriate Health and Social Care services that support their recovery and support them in accessing these services.
• To lead and participate in the clinical activities of the Team such as evening service and duty worker system.

• To work as an effective member of the multi-disciplinary team ensuring effective communication with other team members and other professionals.
• To attend, lead and contribute to multi-disciplinary and multi-agency clinical meetings.
• To receive and participate in clinical supervision in accordance with Trust policy
• To attend, lead and contribute to team meetings.
• To maintain electronic health care records in accordance with Trust’s Record Keeping policies.
• To record data in line with the service Operational Policies
• To produce high quality letters and reports about service users’ care and treatment.
• To use Information Management and Technology effectively and efficiently in accordance with Trust policies.
• To complete incident reporting in accordance with Trust policies
• To participate in activity monitoring & basic audit as required
• Ensure individual readiness, and participate in team readiness, for meeting all applicable CQC quality standards
• To initiate and participate in Quality Improvement (QI) projects in the service.
• To undertake Trust mandatory training and appropriate training identified through supervision and appraisal.
• To identify own training, educational and development needs through appraisal and participate in a personal development plan to meet identified needs.
• To provide support, guidance and supervision to junior staff as appropriate.
• To provide mentorship to students/volunteers/apprentices on placement.
• To participate in training within the Team and to other professionals and agencies.
• To liaise with Estates & Facilities regarding facilities used by the service
• To report to line manager any issues that are of concern relating to health & safety of the building
• Work in accordance with the appropriate ELFT policies and procedures at all times.

• Work flexibly within an agreed number of hours to meet the demands of the service from Monday to Sunday 8am to 8pm.
