Nursing Associate- Hillingdon District Nursing

As a Nursing Associate working within a District Nursing team in the Hillingdon community, you will provide nursing interventions to a range of housebound patients and help to manage their care supported by a team of highly skilled nurses. We are proactive in looking for innovative Nursing Associates who will facilitate both the implementation and development of new ways of working and want to progress their career. We welcome applications from experienced, newly qualified, or soon to qualify Nursing Associates.

Main duties of the job
We are looking for Nursing Associates to work in our District Nursing teams over a seven-day period with set shifts as follows: 08.00hrs - 16.00hrs, 10.00hrs - 18.00hrs, or 08.00 - 18.00hrs. Our teams are led by our incredibly experienced and supportive District Nurse Team Leads, all of whom obtain their specialist qualification in District Nursing. You will work alongside your District nursing team colleagues and an array of specialty teams such as our Care Connection Team, Palliative Care Specialist team, Tissue Viability Team, Rapid Response Teams, Occupational Therapists, and Physiotherapists, all of whom work collaboratively to ensure individualized patient-centered care.
Job responsibilities
* Understand and act in line with NMC professional standards for practice contained within the Standards of Proficiency for Nursing.
* Manage a delegated active caseload, working as a member of a skill mixed team, responsible for interacting constructively with team members and organizing, prioritizing, and planning their work.
* Use a range of assessment tools pertinent to the patients' needs to inform the assessment and assess risk for both patients and staff.
* Understand when to seek appropriate advice to manage a risk and avoid compromising quality of care and health outcomes.
* Demonstrate the ability to effectively and responsibly access, input, and apply information and data using a range of methods including digital technologies.
* Demonstrate an ability to prioritize and manage their own workload and recognize where elements of care can safely be delegated to other colleagues, carers, and family members.
* Contribute to the provision of care for people, including those with complex Patient Care.

Skills and Knowledge
* Ability to work effectively as a team player under appropriate supervision and as part of a multi-disciplinary team.
* Insight into how to evaluate own strengths and development needs, seeking advice where appropriate.
* Understand the role of the Nursing Associate.
* Evidence of time management skills and ability to prioritize.
* Ability to communicate with members of the public and health and care providers.
* Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact.
* Ability to move between sites working across health and social care as required by the needs of the development programme.
* Ability to work on own initiative.
* Ability to take part in reflective practice and clinical supervision activities.
* Knowledge of when to seek advice and refer to a registered care professional.
* Understanding of the importance of the promotion of health and well-being (Making Every Contact Count).
Education and Qualifications
* GCSEs Grade A-C in Maths and English or key skills level 2 in Maths and English.
* Demonstrable ability to study at Level 5 Diploma of Higher Education Level and commit to completing the Foundation Degree programme.
* Qualified Nursing Associate.
* L3 qualification in health or care-related qualification e.g. NVQ Care certificate or equivalent.
PREVIOUS EXPERIENCE
* Experience of working in a health or caring setting.

Salary: £31,081 to £33,665 a year (pro rata if P/T)
