Band 6 (Crisis Resolution and Home Treatment Team - Early Discharge Team).
Are you a dynamic, confident and motivated Mental Health Nurse/Occupational Therapist or Social Worker with excellent communication skills and the ability to assess and lead on clinical decision-making? If so, we would really like to hear from you.
This is an exciting role, working in a fast-paced environment within a well-established, multi-disciplinary team. The crisis and home treatment teams offer a high-quality assessment and home treatment service as an alternative to hospital admission, to individuals experiencing acute mental health crises.
As a service, we are looking for individuals who are genuinely warm, engaging, and focused on client/customer satisfaction. Secondment opportunities/fixed-term contracts for up to 12 months are available. We also offer full-time and part-time working.

Main duties of the job
* To work in collaboration with the acute inpatient wards to facilitate safe and positive early discharges.
* To provide first-line assessments of people experiencing acute mental health difficulties and act as gatekeepers to acute admissions.
* To rapidly assess people with mental health problems who are vulnerable to hospital admission.
* To provide specialist crisis assessments focusing on 'hospital at home'.
* To participate in team MDMs.
