Job summary
Workingin accordance with the NMC Code of Conduct, the post-holder will provide full Practice Nurse duties including assessment, treatment, screening, health education services and advice topatients.
Thepost-holder will work with reference to practice agreed protocols andguidelines, providing nursing treatments to patients both independently and in participationwith nurse colleagues and general practitioners.
Thepost-holder will have the required level of training and competence and willwork within patient group directions where these are available.

MAIN DUTIES OF THE ROLE
All Practice Nurse duties including;
Management of chronic disease
1. Identifying significant abnormalities
2. Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
Patient health checks
3. Identifying significant abnormalities
4. Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues
5. Working with patients to develop a management plan where health problems or potential health problems are identified
Risk assessment
6. Recognising issues and gathering sufficient information to refer ( drugs; domesticviolence; child protection; vulnerable adults; senior patients; social problems)
Health screening
7. Recognising issues and gathering sufficient information to refer ( womens and mens health, sexual health, older people)
8. Undertaking monitoring tasks ( smears) and providing advice as appropriate
9. Providing in-depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans.
Travel health
10. Administering injections and providing guidance in accordance with guidelines
11. Identifying problems
12. Provision of specialist and evidence-based support to the team.
Immunisation (adult and child)
13. Administering appropriate immunisation autonomously
14. Working with patients with more complex immunisation issues.

15. Working independently or alongside the general practitioner and making decisions/ recommendations as appropriate.
You will be expected to maintain up to date registration and ensure mandatory training is kept up to date.

In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential
18. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
Health & safety:
The post-holder will implement and lead on a full range of promotion and management of their own and others health and safety and infection control as defined in the practice health & safety policy, the practice health & safety manual, and the practice infection control policy and published procedures. This will include (but will not be limited to):
19. Using personal security systems within the workplace according to practice guidelines
20. Awareness of national standards of infection control and cleanliness; regulatory / contractual / professional requirements; and good practice guidelines
21. Responsible for the correct and safe management of the specimens process, including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
22. Management and maintenance of Personal Protective Equipment (PPE) for the practice, including provision, ordering, availability and ongoing correct usage by staff
23. Responsible for hand hygiene across the practice
24. Ownership of infection control and clinically based patient care protocols, and implementation of those protocols across the practice
25. Active observation of current working practices across the practice in relation to infection control, cleanliness and related activities, ensuring that procedures are followed and weaknesses / training needs are identified, escalating issues as appropriate
26. Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process
27. Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes
28. Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand-cleansing facilities, wipes etc. are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.
29. Safe management of sharps procedures including training, use, storage and disposal
30. Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile way, safe and free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management
31. Actively identifying, reporting and correcting health and safety hazards and infection hazards immediately when recognised
32. Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers
33. Undertaking periodic infection control training (minimum twice annually)
34. Routine management of own team / team areas, and maintenance of work space standards
35. Waste management including collection, handling, segregation, container management, storage and collection
36. Spillage control procedures, management and training
37. Decontamination control procedures, management and training, and equipment maintenance
38. Maintenance of sterile environments
39. Demonstrate due regard for safeguarding and promoting the welfare of children.
