You will provide holistic nursing assessment, high quality care, support, advice, and information to patients and carers with the aim of maintaining patients in their own homes, enabling them to enjoy maximum independence and optimum quality of life.
Ensure the effective co-ordination and provision of the nursing service in the absence of the Locality Nurse Lead and to actively participate in the planning and discharge of complex patients from secondary to primary care.
Working across the Intermediate care and community nursing teams to provide crisis response nursing interventions, assessment, liaison, referral, co-ordination, delegation of care, or placement in order to prevent hospital admissions
Part time hours will be considered.
Main duties of the job
1. To deliver high quality care to the housebound patients and work as part of multi-disciplinary team and in partnership with primary care and secondary care.
2. To provide specialist clinical advice to the multi-disciplinary team
3. To provide initial and ongoing assessment of nursing needs of patients and carers in the community. When planning a programme of care, physical, social and emotional and cultural factors are taken into consideration and suitably qualified staff used to facilitate the level of care agreed
4. To work as an autonomous practitioner in the planning and implementation of nursing treatment programmes
5. To advise and instruct carers and patients on health and wellbeing issues, and self-care strategies
6. To carry out specialist and shared complex nursing assessments, to include continuing health care, medication review, prescribing and palliative care
7. To utilise a broad range of specialised nursing skills and knowledge aligning practice to the organisations Medicines Policy for Registered professionals and the NMC Standards for Medicines management (2007) ensuring assessment and competence is maintained and recorded as a nurse prescriber
8. Supporting the Zone Lead with staff supervision, appraisal's and sickness management
