Stroke Service -Assistant Practitioner Band 4- Louth/East Coast (other areas considered)
Are you passionate about delivering a high quality service to patients who have had a stroke in Lincolnshire? Are you motivated and dedicated to supporting patients with the transition from hospital to home, and to continue with their rehabilitation in the community? We are looking to recruit a highly motivated Assistant Practitioner to join the service in these exciting new developments.
The innovative Stroke Service provides high quality specialist stroke rehabilitation in the community across Lincolnshire. The service has four bases across the county and works as a very flexible team. This post will require the ability to travel to meet the needs of this county wide service and applicants must be able to work on a rota basis to assist in covering a 7 day a week 8am to 6pm service.
You will play a key role in supporting Therapy staff in delivering high quality rehabilitation to people who have recently experienced a stroke.
Please note a foundation degree in healthcare (or equivalent) is essential for this post.
A Band 3 to Band 4 developmental post will be considered for where the applicant is unable to demonstrate the required experience within their interview. Appropriate qualifications for the role still required
Please note this position will not commence before July 2025
Experience of working with people who have had a stroke would be an advantage but is not necessary as training will be provided. We would also welcome applications from individuals who have not worked with all three therapies as training in each discipline will be provided. We have a comprehensive in-house training programme structured around core stroke competencies, regular in-service training in teams, and access to further learning through LCHS and external courses.

DUTIES
The post holder will participate as a member of the multidisciplinary team in the assessment, planning, delivery and evaluation of patients/clients health and well being needs
To accept delegated assessment responsibility, developing individual plans of care to meet the immediate and ongoing needs of the service user and carers.
To have knowledge of specific physical, emotional, psychological, social, cultural and spiritual needs of the service users and carers and, taking these into account, assist in the implement appropriate action to meet those needs
Plan and progress patient interventions with autonomous assessing and planning on a daily basis with indirect supervision of qualified practitioner
Teach patients/carers with the indirect supervision of a qualified practitioner
Promote a positive image of the organisation, department and service.
To be responsible for the safe use of all equipment within their scope of practice.
