ALCOHOL WORKER (Community Alcohol Care Team – CoACT) Cwm Taf Morgannwg Job Description Role Alcohol Care (Community Alcohol Care Team – CoACT) Service Cwm Taf Morgannwg Location: Cwm Taf and Bridgend Barod Base Barod bases across CTM: Oldway House, Merthyr Tydfil. Engine House, Aberdare. Celtic Court, Bridgend. Hours: 37 per week Salary: £25,750 (starting point) Reports to: Engagement Team Leader Team Members Reporting to Job Holder: NA Contract: Permanent in line with funding. The Service CTDAS and BDAS provide a service to people who use drugs or alcohol, their families, and communities. This will include early intervention, engaging and assessing service users, needle exchange, brief interventions, outreach, training, case management, health promotion and harm reduction initiatives and recovery and aftercare services. Role Purpose The purpose of this role is to provide community-based alcohol interventions to people who have been assessed in hospital by the Alcohol Care Team (ACT) following alcohol related hospital admissions. Postholders will carry out assessments and interventions predominantly in people’s own homes as well as other community-based settings. This role will work closely with the hospital ACT to ensure appropriate procedures are in place for community discharge, and handover of ongoing treatment needs/ recovery. Main Duties and Responsibilities Conduct initial assessment and care planning and provide access to a range of interventions. To support people within the service holistically to ensure all support and treatment needs are met. Undertake risk assessment and develop and review risk managements plans that seek to mitigate identified risk. Provide structured support to service users experiencing issues with alcohol that do not require referral for relapse prevention medication. To utilise psychosocial interventions, motivational interviewing and cognitive behavioural techniques where appropriate to promote engagement in treatment services, encourage self-esteem, well-being, self-responsibility and enhanced motivation. Understand and use pathways (for detox and rehab services) available to those who use alcohol. To actively engage with service users in the community and work to re-engage them in treatment services if required. Support service users to attend initial appointments or appointments with other health professionals. To work within agreed policies, procedures and protocols to safeguard children, vulnerable adults and those experiencing domestic abuse. To work in partnership with other agencies to provide a package of support around holistic needs, by attending joint allocation meetings and case reviews. Enter data onto the case management system Palbase, and use the system effectively to inform work with service users, and undertake relevant administrative functions. Contribute to the development and review of organisational policies and procedures as and when appropriate. Positively promote and represent CTDAS and BDAS in the public / external arena including engaging with media. Retain and maintain all information (paper and electronic) in line with service level agreed guidelines and statutory protocols, such as GDPR. Essential Desirable Qualifications and training Full, current driving licence and use of a vehicle which is insured for business use. Level 3 in Advice and Guidance (or equivalent). Training in any of the following areas: Safeguarding Boundaries Substance Awareness Harm Reduction Safer Injecting Psychosocial Interventions Role-related knowledge, skills & experience Experience of working in an outreach role within the community. Understanding of alcohol care pathways in the community. Ability to work with people regardless of their race, gender, age, religion, disability, identity, sexual orientation and experience. Time management skills. Literacy and numeracy skills. Excellent IT skills: competency in Microsoft Word, Outlook, Teams, Excel, PowerPoint, Canva and Internet Explorer.
