Our team cover the geographical area across Yeovil, Wincanton and surrounding areas. Your base will be Wincanton role of the district nurse has changed over the years, and we are now looking after patients in their home environment including complex wound management, palliative care and end of life care, medicines management, as well as having a high standard of holistic assessment and care planning will be working independently to support with managing the care of patients in our local community and wider are recruiting due to staff development and promotion - as a service we pride ourselves on colleague development and succession planning. Main duties of the job
Shift patterns include: 8am-4pm, 9am-5pm, 10am-6pm, 2pm-10pm. To include one weekend in Somerset, we're working with health and care partners with a newly developed Hospital @ Home programme to care for patients at home safely, rather than being in service is a safe and efficient alternative to hospital care that is enhanced by technology. It supports patients to remain at home, who would otherwise be in hospital to receive the acute care, monitoring and treatment that they includes either preventing avoidable admissions into hospital or supporting early discharge out of hospital. The service can help patients to complete their treatment or be monitored prior to surgery from the comfort of their own home.

1. Registered General Nurse
2. Level One Registration with appropriate registration with the NMC as per post requirements
