We are excited to have an opportunity for a band 6 Community Mental Health Nurse in our Young Onset Dementia service in Doncaster.
We are looking for someone with a passion for working with this client group and who brings advanced knowledge and experience to the role.
You will deliver an evidence-based service, contributing to the delivery of assessments, individualised interventions, and discharge planning as part of an effective, flexible, compassionate, and patient-centred approach for people with living with Young on Set Dementia.

The post holder will be responsible as the Lead Professional for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers as part of a multidisciplinary approach. To provide professional advice and support to other agencies and members of the multidisciplinary team.
The post holder will provide supervision and appraisal of other staff and students and professionally lead and supervise and support staff. They will be supported by regular supervision and ongoing CPD opportunities.
