Job summary As a PCN Clinical pharmacist you will have an exciting opportunity to develop these new roles in practices and work within a team of clinical pharmacists supported by mentoring and supervision from a senior experienced clinical pharmacist and a GP supervisor. Working as part of a multi-disciplinary team, you will develop and run processes for repeat prescription re-authorisation, acute prescription requests, management of medicines queries on transfer of care and systems for safer prescribing. Also, the successful candidate will have a role in managing long-term conditions and will be expected to perform face-to-face medication reviews of patients on quality improvement and clinical audit. You will also provide primary support to general practice staff, across the PCN. Main duties of the job The candidate will provide proactive leadership on medicines and prescribing systems to the PCN multidisciplinary team, patients and their carers. You will also develop and deliver patient facing services and clinics which improve patients' outcomes, reflect the needs of the PCN population and shift workload from other disciplines. You will deliver direct patient consultations and prescribing as part of the practice and cluster multidisciplinary team approaches acute illness.Working closely with PCN Pharmacy colleagues, organize and oversee the PCNs medicines optimisation system including the repeat prescribing and medication review systems.
