An exciting opportunity to join the Discharge to Assess (D2A) team at Barnsley NHS Foundation Trust as a Band 3 Occupational Therapy Assistant.
The established service provides a clear pathway from the acute hospital by assessing the patients' therapy and care needs in their own home. This means that the patients no longer need to wait in hospital for these specific assessments, which will reduce delays in discharges, improving patient flow and allowing patients to go home earlier.
The team is based at Kendray Hospital therapy hub, involves interdisciplinary working alongside health/social community care and therapy support services. The post holder would be expected to work within 08:00-18:00 service and contribute to a 7-day service. They should be able to work independently and within a team, providing high-quality care in a fast-paced working environment. Applicants should possess excellent interpersonal and communication skills to work collaboratively with colleagues and patients.
The team fosters a supportive environment that seeks to develop therapy staff and provide them with the skills to work within the D2A model in the home environment.
Main duties of the job

Due to the nature of the role, the successful candidate would be required to be a car driver with access to a car for work purposes.

We depend upon our non-registered workforce to support the work of our registered practitioners, which ranges from attending home visits together, to treating patients individually with programmes delegated by the occupational therapist, or providing and fitting key pieces of equipment to ensure the safety of the individual.
The role requires individuals who are passionate about good quality care and seeing great outcomes for the people that they support. You need to have a compassionate approach and good communication skills to work with a range of patients and medical conditions, including vulnerable adults.
The role includes using and documenting in the patient record system (Systm1) and liaising with other services within the acute hospital Trust and the community.
