We are seeking a highly motivated and compassionateCommunity Psychiatric Nurseto join our multidisciplinary team.
You will play a crucial role in providing holistic care to individuals with diverse mental health needs within the community.
Key Responsibilities:
• Comprehensive Care Management:
• Conduct thorough holistic assessments of individuals' mental and physical health needs, social circumstances, and support systems.
• Develop, implement, and evaluate personalized care plans in collaboration with service users and the multidisciplinary team.
• Coordinate and deliver a range of therapeutic interventions, medication management, and practical support.
• Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
• Arranging, contributing to and documenting care reviews, including telephone, home visits, video and face to face reviews – as appropriate and defined within the care group SOP
• To supervise junior staff members and to support students in placement
• To work collaboratively with the multidisciplinary team as well as patients, carers, family members and internal and external agencies.
• Effective communication skills and self-governance are required. You will be able to demonstrate knowledge of high-level interventions & community assessment skills. Existing qualifications or a willingness to undertake training relevant to the post is expected.

• Recording and reporting accurately any changes related to the patient’s current behaviour and/or risks to the Nurse/healthcare professional in Charge in a timely manner.
• Recording care given and reporting omissions accurately and in a timely manner.
• Supporting and carrying out therapeutic observations as required and instructed.
• To make recommendations to the care plan process
• Promoting a healthy, safe and secure environment by adhering to Trust statutory health and safety requirements and maintaining confidentiality of information at all times in compliance with Trust policies, procedures, protocols and guidance
