Provide tailored advice, guidance, and practical support to young people leaving care. Work closely with young people to develop individual pathway plans, addressing areas such as housing, education, employment, health, and emotional wellbeing. Advocate on behalf of young people to ensure their needs are met by other services and professionals. Foster a supportive, non-judgmental, and empathetic environment that encourages young people to develop their skills and resilience. Help young people to access resources, opportunities, and support networks to aid their transition to independent living.
