About The Role
The main duties of a Social Worker in the Hospital Social Work Team include:
1. Supporting an integrated discharge hub in ensuring that residents are safely discharged with appropriate support in place.
2. Supporting the completion of Mental Capacity Assessments for residents admitted to Hospital in relation to decision-specific issues.
3. Ensuring that residents’ human rights are upheld and their right to a Private and Family Life is at the centre of our intervention.
4. Working jointly with health colleagues to triage referrals from Hospital Wards for residents identified as requiring support on discharge.
5. Completing Safeguarding enquiries for residents that have been subject to abuse or harm and following Pan-London Guidance.
6. Working as part of a duty service to respond to immediate issues on a regular basis.
7. Work collaboratively with residents, families and professionals to help co-ordinate appropriate discharge planning for residents.
8. The role is based on-site at Percy House, West Middlesex University Hospital and as such attendance in person on a daily basis in the office will be required.
9. You will be part of a weekend duty rota, which is rewarded with a market supplement in pay.
About The Team You’ll Be Working In
The Hospital Social Work Team is a fast-paced person-centred service that places the needs of our residents first. We have a strong team work ethic and attend the Hospital wards on a daily basis to spend valuable face-to-face time with our residents prior to their discharge. We fundamentally believe that all our residents are entitled to the best level of care and support and work tirelessly with our colleagues in the NHS to support safe discharges. About You
You will have contributed to a positive team culture and be a strong team member, with an emphasis on achieving collective goals and supporting colleagues on a daily basis.
You will bring experience of delivering positive outcomes for residents that you have worked with and demonstrate examples of where you have upheld human rights.
You will have sound and detailed knowledge of legislation such as the Care Act 2014, Mental Capacity Act 2005. You will require a good knowledge of Ordinary Residence statutory guidance and Safeguarding Adults procedures to ensure that you meet the needs of the residents we work with on a daily basis.
You’ll have to demonstrate your ability to work to tight timescales in a fast-paced environment and be able to flexibly manage your day-to-day priorities.
Qualifications:
Social Work qualification required and/or awaiting certificate from Social Work England.
