Your role and responsibilities support the Home First Service within a locality. Completing care visits as required and to assist with Short Term Reablement Plans (STEP), to contribute to assessment and reablement need to mentor and direct new staff across the team to support their work and help them develop in their role be liaising with multi-disciplinary teams to support the customers within your team You will work alongside team colleagues and be supporting the Home First team Leader for your area and pick up tasks as directed such as medication observations and issues, training basic skills to new staff be a risk assessor for the team, working with the customer initially to identify any risks to them to ensure we can care, support and re-able people in a safe way the senior point of contact for Reablement Support Workers in the team for initial advice and guidance “Trusted Assessor” trained, which will enable you to order small pieces of equipment for our customers to help them remain independent involved in front line care delivery and at times, assist customers with dignified and intimate personal care needs engage with customers, family carers and others in their network be involved in more complex care packages whilst always having the right training and wider support, for you to carry this support out confidentially and safely. promote alternative ways to meet and maintain their long-term needs promote a range of services to help people live independently and safely at home work with our assessment staff on how customers are progressing with their goals and outcomes complete records for the ongoing review process development opportunities into specialisms and be champions in your team for a variety of different skillsets. You will need ideally you will need an NVQ level 3 or willing to work towards the ability to remain calm in stressful situations to be competent in use of email and mobile applications need to have a passion for working as a team, good verbal and written communication skills, to support team colleagues to effect high quality outcomes for customers.
