Care Coordinator - Dene & Stour Valleys Primary Care Network
The Dene and Stour Valley PCN seek an enthusiastic and proactive Care Coordinator to join their Wellesbourne team. The Dene and Stour Valley is a patient-focused, forward-thinking PCN looking for people to work with the team to deliver care to their patient populations. The Practice Coordinator role is a new and evolving role within NHS Primary Care Networks. The post holder will require a flexible approach to work; the role will consist of some focused project work as well as general core support to the PCN. The post holder would ideally have good knowledge of the community or primary care sector and strong IT skills as the role will be key to practice searches and audits.
Location: The successful candidate will be based at Hastings House Medical Centre, but must be willing to travel between Dene & Stour Practices if/when required to support the business needs. Due to the geography and nature of the role, a clean UK driving license and full use of a car is essential.
Main duties of the job
Practice Coordinators provide extra time, capacity, and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals. They will support the Clinical Directors in coordinating all key activity including improving access to services, providing advice and information, and ensuring health and care planning is timely, efficient, and patient-centered. They will work closely and in partnership with the Social Prescribing Link Workers and support delivery of the comprehensive model of care to reflect local priorities, health inequalities, or population health management.
Job responsibilities
Care Coordinators will:
1. Provide coordination and navigation for people and their carers across health and care services, working closely with Social Prescribing Link Workers, Health and Wellbeing Coaches, and other Primary Care roles.
2. Work alongside the Health & Wellbeing coaches to identify patients that require additional one-to-one support, particularly with mental health issues, and provide this support.
3. Liaise with Mind Support workers, GPs, and Social Prescribing Link Workers to identify patients that would benefit from one-to-one support and provide this using the available decision support aids.
4. Feedback appropriate patient information to Place Based Teams to ensure all services can support the timely care of the patient.
5. Help people to manage their needs and support them to make appointments.
6. Raise awareness of shared decision-making and decision support tools and assist people to be more prepared to have a shared decision-making conversation.
7. Ensure people have good quality information to help them make choices about their care.
8. Support people to understand their level of knowledge, skills, and confidence (their “Activation” level) when engaging with their health and wellbeing, including through the use of the Patient Activation Measure.
9. Assist people to access self-management education courses, peer support, or interventions that support them in their health and wellbeing.
10. Support the coordination and delivery of MDTs within PCNs.
11. Collate agendas, produce minutes/action logs for PCN/MDT meetings, and ensure all actions are completed, including follow-up if necessary.
12. Collate and manage shared documents on the appropriate platform, ensuring the latest versions are available and made available in a timely manner.
13. Engage in specific project work liaising with practice leads on the implementation of shared processes and collaboration opportunities to support improved patient care.
Person Specification
Qualifications
* Demonstrable commitment to professional and personal development.
* NVQ Level 3, Advanced level, or equivalent qualifications or working towards.
* Training in motivational coaching and interviewing or equivalent experience.
Experience
* Experience of supporting people, their families, and carers in a related role.
* Good experience of IT systems and packages including EMIS Web and Docman 10.
* Experience of data collection and providing monitoring information.
* Experience of working directly in a community development context, adult health and social care, learning support, or public health/health improvement (including unpaid work).
* Experience of supporting people with their mental health.
* Experience of collaborative working and building relationships across varied organisations.

£25,112 to £28,348 a year, dependent on qualifications & experience.
