A fantastic opportunity has arisen for an efficient, organised and experienced Care Coordinator/Supervisor to join a reputable, well-established and rapidly expanding Domiciliary Care Business in North London. You will need to provide support for vulnerable client groups within their own homes and specialise in elderly, dementia, learning disabilities and physical disabilities.
About the Role
* Dealing with queries from clients, families and public representatives in a polite and professional manner.
* Line management and supervision of Domiciliary Care Workers and learners who are shadowing the job while on training.
* Ensure that the domiciliary care worker rotas are organised and sufficiently well planned taking into account travel time during rush hour, city charges and routes.
* Liaise with internal and external company stakeholders in respect of developing and delivering holistic, person-centred packages of care.
* Carry out on-call coordinating and response duties outside normal office hours as per the contract.
About You
* Must have experience within a domiciliary care coordinator position.
* You must have valid training care certificates (completed within the last 12 months) and/or a related degree in Social Care Services.
* Background within rostering and using software such as Care Line Live, Nourish (or similar).
* Due to the vulnerability of the client group, you must have a clear enhanced DBS.

Job Description
1. Care Coordinator will work closely with the Registered Manager and will be the main contact for the organisation for new referrals, responses to new referrals, accepting new referrals within agreed time, opening the Service User Files and uploading the information on the software, monitoring and supervising the Field Care Supervisor with the above duties.
2. Accepting, allocating and processing referrals for new work efficiently.
3. Roster geographically to maximise utilisation of resources.
4. Accepting and processing changes and amendments to existing care packages.
5. Assisting the Field Care Supervisor directly responsible for new referrals, rota, scheduling, sickness and holiday cover arrangements requested by the Business Lead to assist the Operations Team.
6. Assisting the Operation Team in DN, OT or Increase request referrals in liaison with Training and Assessment Coordinator.
7. Assisting the Registered Manager with Complaints and Safeguarding Investigations if and when required.
8. Ordering, recording and where appropriate the administration of prescribed medication and ordering the incontinence pads if and when required.
9. Coordinate 2 weeks rota and plan ahead to minimize risk of staff shortage.
10. Ensuring care worker rosters are completed on a timely basis for the week.
11. Complete weekly report on KPI’s lateness/missed calls/sickness/hospital admissions/complaints.
12. Cover Out of Hours calls on a rota basis.
13. Monitoring and planning the Service User and Staff Visits and Telephone Monitoring weekly in liaison with the Office Administrator.
14. Regularly assisting the Administrator in the preparation of Service User Files, Staff Files, and Application Forms etc.
15. Checking incoming emails every 30 minutes and ensuring everyone takes the necessary responsibility and accountability to respond to the emails on time.
16. Carrying out their own administrative duties for the day-to-day management of the agency, such as filing, pulling of records, word processing, copying, faxing, checking emails, and preparation of documents.
