Band 6 Community Psychiatric Nurse - Assertive Outreach Service

An exciting opportunity has arisen for a Band 6 CPN looking for a new challenge to work within East Assertive Outreach Team.
The Assertive Outreach Service is a multidisciplinary team that adopts a person-centred approach, focusing on individual recovery. We work with adults with complex, severe, and enduring mental health needs in the community, supporting individuals who have historically avoided contact with mental health services using a flexible and creative approach to engagement.
This position is suitable for a Registered Mental Health Nurse with a degree-level qualification or equivalent and registration with a relevant professional body (i.e., NMC) with several years of community experience working with service users with complex long-term needs.
The Assertive Outreach service operates over 7 days a week, 365 days a year. Our shift patterns include day shifts (9:00 am - 5:00 pm), late shifts (12:00 pm - 8:00 pm), and weekends (9:00 am - 5:00 pm). Flexible working arrangements can be offered during these hours.

Main Duties of the Job
* Act as Key worker to a prescribed caseload, effectively managing your workload utilizing Dialog plus care planning tool in recovery-focused interventions, medical reviews, and developing a therapeutic relationship with the caseload.
* Work as an independent practitioner and collaboratively as part of the Team, demonstrating the ability to handle challenging and complex issues while joint working with other professionals to support service users and families.
* Demonstrate a range of skills including effective communication, empathy, self-awareness, clinical skills, understanding of the Mental Health Act, compassion, and critical self-reflection.
* Attend daily handovers, maintain high levels of communication within the team, and contribute to the risk management processes within the service.
* Possess a thorough understanding of the role of a CPN and the key worker role, articulating this in practice while demonstrating high levels of skill in assessment, intervention, and planning within a client-centered recovery approach.

Person Specification
Qualifications
* Degree or Diploma in Mental Health Nursing
* Current NMC Registration
Experience
* Experience in adult mental health settings
* Supervision of junior staff and/or being a placement educator
* Demonstrated experience of MDT working and collaboration with external agencies/healthcare providers
* Community experience
* Demonstrated leadership skills
Knowledge
* Knowledge/experience of the recovery approach in mental health
* Knowledge/experience of clinical audit
