Woodfield 24 Care Services is looking to recruit an experienced, enthusiastic, and forward-thinking individual with strong leadership skills to fill an existing key position within our organisation.
Our Assessment Officer will oversee all aspects of the Woodfield 24 End of Life care service ensuring compliance with the CQC’s Fundamental Standards and Woodfield 24 care services policies and procedures.
Through effective leadership you will provide an innovative approach to managing a care service while promoting a culture of compassion for both staff and service users.
You will be at the forefront of the co-ordination of care service and roles, undertaking the lead role in identifying and implementing new packages of care and the day-to-day management of accepting patients, arranging prompt assessments, liaising with relevant professionals, and carrying out all in line systems, and processors related to care delivery to our patients at the end of life.
You will provide direct line management and carry out observation and competency assessment to all care staff within the agreed areas of responsibility to ensure our people feel fully supported and to allow Woodfield 24 to retain its reputation as the very best community-based palliative care provider in the region, ensuring standards of care are maintained at the highest level.

Qualifications
Essential - Professional Qualification in health and/or social care. Ideally the candidate would already hold a qualification or equivalent experience at minimum Level 3.
Experience/Knowledge
Essential - Demonstrable experience of leading patient care co-ordination, providing team leadership, demonstratable flexibility to promote service delivery, Hold a NVQ 3 within a health or social care setting; extensive experience of working with Carers; experience of having managed led staff groups, including supervision, Experience of managing and facilitation care for numerous patients in a past faced environment, knowledge of Social Care policy, and its impact on service delivery; evidence of continuous personal and professional development; understanding of CQC’s Essential Standards of Quality & Safety; knowledge and experience of providing and leading on person centred care and support packages to meet individual need; experience of managing employee related matters; knowledge and experience of risk assessment processes. Flexible and adaptive to change.
Desirable - Specific care leadership experience; experience of management of change; experience of safeguard reporting; an understanding of the inspection process for services registered with CQC; understanding of compliance in care and governance framework knowledge of their role within it; experience of staff and service user engagement; Positive team player with vision for enhancing holistic care.

Essential - Well-developed leadership skills; good communication skills and the ability to communicate sensitively; ability to maintain working relationships across involved agencies and within Woodfield 24; to encourage and motivate staff to work towards the provision of safe, caring, responsive, effective, well led care; ability to set clear standards of performance and work priorities for self and others; ability to understand and work within the guidelines of service policies; ability to teach groups of staff and provide leadership; full IT competency; ability to work to agreed deadlines. To have a flexible approach to role and willingness to adapt quickly in situation of service need. Ability to bring new ideas forward in becoming holistic and flexible for patients in need.
Desirable - Preparation of user accessible information in a variety of formats; experience of audit process.
Essential - Ability to travel within the entire geographical work area day or night based in the Doncaster area, ability to complete assessments for patients and provide personal support to staff in other areas when required. Willingness to complete on call duties daily and the ability to cover on call/working at night (when required).

• To act as an on-call support- Day, Evening and Night (when required).
