Job summary An exciting opportunity has arisen for an enthusiastic, skilled and experienced Rehabilitation Coach to join the Live Well rehabilitation team. Working in partnership with leisure and third sector organisations, this therapy team supports people living with Long Term Health Conditions to achieve better health outcomes and quality of life. The post holder will deliver group based exercise, activity and nutritional programmes in leisure and community centres across Cardiff and Vale, aimed at supporting people to self manage their health condition. Main duties of the job Applicants will have relevant qualifications to the role and knowledge of health promotion, long term condition management and experience of group facilitation. They will have experience in health promotion type activities and demonstrate strong communication and interpersonal skills. Previous experience of groups delivery, data collection and audit are desirable. Supporting people with long term conditions to meet their goals in a number of community settings including groups and 1:1 under the guidance of the registered staff. Leading group activities. Delivery of online and face to face interventions. Collection of data and service user administration. Previous experience as a therapies support worker or experience as an exercise professional is advantageous and the ability to work well within an interdisciplinary team is essential. You will need to have excellent verbal and non-verbal communication skills, be able to work as part of a varied team, and be able to motivate service users. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
