An exciting opportunity has arisen to offer a fixed term / secondment one full time and one part time role as Placement Coordinator to support the Transfer of Care Hub based at St Margaret's Hospital in Epping.
This post is to work with a team of staff covering 7 day shift pattern with hours of work 08:00am-18:00pm. This is funded until 31 March 2026.
You will be part of a multidisciplinary team working to deliver the vision of the One Health and Care Partnership improving people’s outcomes and experience by navigating to services at the right time, in the right place.
Working collaboratively with Essex County Council Service Placement Team in sourcing placements or provision of care and with Clinical staff, Social Workers and Occupational Therapists to deliver bespoke intervention work including assessment of risk through direct work, within a quality assurance process.
To receive referrals by telephone or electronically which are recorded onto CCC digital patient tracker and EPR.
To perform the initial checks of the electronic referral for comprehensiveness and accuracy.
To gather a current holistic overview of the adults’ health and care journey. This will inform the decision making around the complexity and priority level of the referral.
Following an appraisal of the adult’s needs the referral will be directly referred to a service, or where there is any doubt surrounding next steps, you will escalate to the CCC Professional Lead to step in to make the decision.
To escalate issues related to actions which remain unresolved or complex to the Professional Lead in a timely manner.
Following each referral received, to update the EPR and patient tracker with all appropriate data relating to the adult to maintain a live overview of system capacity and adult transfers of care.
To provide up-to-date information to the triage team, professional lead and MDT team for any system escalation calls and provide updates to system partners when requested.

To undertake the allocation / prioritisation of daily support for the MDT team
