CLCH BRENT RAPID RESPONSE - SENIOR PHYSIOTHERAPIST
We are seeking an enthusiastic Specialist Physiotherapist to join our Unplanned Care team based at Sudbury Primary Care at Wembley, London, delivering care to patients closer to home and preventing unnecessary hospital admission in Brent.
This role will primarily work under the Rapid Response pathway but may require the candidate to work across the other pathways when required. All pathways deliver nursing and therapy care in a patient’s usual home environment.
RR focuses on preventing avoidable hospital admissions for patients with acute ill-health conditions by providing holistic assessment within 2 hours of referral and treatments over a 5-day pathway. The pathway is delivered in conjunction with Adult Social Care to support the patient’s reablement.
The Unplanned Care team works in an integrated way with the other community providers in Brent to ensure holistic support is provided. Just as we care about our patients’ wellbeing, we care about yours!

The successful candidate needs to be a registered Physiotherapist, ideally with advanced clinical skills, significant knowledge, and experience in the clinical care of adults and older people in a range of healthcare settings. Candidates should be self-motivated with strong organisational skills and able to make clinical decisions autonomously as required.
As a clinician, you will use your clinical expertise, levels of judgement, discretion, and decision-making to support patients at home, improve patient outcomes, and reduce the likelihood of unnecessary admissions to hospital. You will be required to complete holistic assessments, giving appropriate interventions as required, and have knowledge of community services to plan the ongoing care of patients.
