Job summary
Working for the Acute Clinical Team (ACT). ACT is an acute service, going into patients' homes and other residence such as nursing homes to assess and treat.
This is a referral service, the aim of which is to reduce hospital admission and expedite early discharge.
Main duties of the job
1. Assess care needs and develop, implement and evaluate the programmes of care without supervision.
2. Delegate the delivery of care where appropriate, in line with NHS Wales delegation framework. Supervise the delivery of care and the staff involved with its provision.
3. Ensure that the patients physical, psychological, social and spiritual needs are met including fundamental aspects of care needs.
4. Monitor and evaluate the standards of care provided by the clinical team.
5. Be visible and accessible in the clinical area to the nursing and wider multi disciplinary team, patients and service users.
6. Continually support and work in conjunction with the Senior Nurse and formally deputise in the absence of the senior nurse.
7. Deal with complex clinical and managerial situations that arise, analysing, seeking appropriate help and making decisions in relation to patient risk and care.
8. Facilitate learning in and from practice.
9. Support a culture for learning and development through promotion of reflective approach to practice and utilising a formal clinical supervision framework in support of team members.

Job responsibilities
10. Combine excellent clinical practice with day to day shift coordination and undertaking leadership and management responsibilities in support of the Senior Nurse.
11. Continually support and work in conjunction with the Senior Nurse including deputising/taking charge of the clinical team in the absence of the senior nurse.
12. Provide supervisory management and leadership skills in the clinical environment which is complex and pressurised.
13. Provide professional nursing leadership and guidance (in line with NMC standards), clinical advice and undertake management and monitoring activities to ensure the effective delivery of high standards of nursing care at all times.
14. Work alongside the nursing team members, providing support to junior colleagues in the provision of direct care.
15. Proactively contribute to the implementation of the NMC standards for education (2018) including the standards of student supervision and assessment.
16. Provide leadership and role modelling in the development and mentorship of new and other staff.
17. Demonstrate the Health Boards' values in everything that you do in the work environment.
18. Support the Senior Nurse in the development of relevant policies, procedures and protocols for the area of responsibility ensuring that they meet national, local and professional criteria, submitting them to them for agreement prior to submission for ratification.
19. Support the Senior Nurse in arranging and leading team meetings.
20. Participate in the investigation of clinical incidents, complaints and serious untoward incidents using root cause analysis methodology.
21. Collate accurate and timely data and information for a range of audit, research and service performance.
22. Take actions continually to both maintain and monitor compliance with the agreed planned rosters.
23. Maintain and develop good interpersonal relationships with clinical specialists from across nursing and wider multi-disciplinary teams, working in partnership to meet nationally set standards ensuring that the risk of harm to patients in minimised.

25. Ensure effective communication with all members of the nursing and MDT, external agencies, patients and their carers/relatives.
26. Provide leadership advice, guidance and clear communication on agreed priorities in the management and delivery of patient care.
27. Ensure all communication and multi-disciplinary approach of patient care.
28. Demonstrate a sound knowledge of department resource costs conscious approach to the use of resources, identifying areas for improvement.
29. Work with the Senior Nurse to monitor the management and expenditure of the budget.
30. Participate in the preparation and monitoring of the planned roster, ensuring cost effective development of personnel whilst working within planned rosters and meeting the clinical care needs for patients.
