Background
Southampton Primary Care Ltd (SPCL) is a federation of Southampton GPs who deliver a wide range of primary care services at scale in the city. Our new Health Inclusion team (HIT) will be working in collaboration with other providers to develop Integrated Neighbourhood teams. The SPCL multidisciplinary Health Inclusion Team will provide targeted population-based interventions to improve health and wellbeing outcomes by minimising the burden of disease and the associated risk factors. We propose a ‘fully engaged’ scenario in which individuals are involved in improving their general health and wellbeing with the emphasis on the prevention and progression of disease.
Our three workstreams are configured to identify and address unmet needs through opportunistic health promotion and health and wellbeing assessments in the community, to optimise vaccine uptake through proactive personalised care planning, telehealth monitoring and to ensure the integration of non-health service providers and digital solutions into the neighbourhood teams.
We are looking to employ experienced Nurse Practitioners (Band 6 and 7) on a full time or part time contract to work across PCN and in the community, the service runs 0800-2200 7 days a week 365 days a year.
This is an excellent opportunity for qualified Nurse Practitioners (Band 6 and 7) to expandtheir knowledge and experience in integrated primary care working.
We would require:
* You to be an Independent Nurse Prescriber or a Nurse Practitioner with experience in history taking, examination and LTC reviews / CGA. Due to SPCL’s developing model of care, primary care experience would be an advantage.
* You to hold a valid driving license.
Your role
* Conducting practice-based clinics and community-based visits to include Comprehensive Geriatric Assessments (CGA), LTC, SMI and LD reviews.
* Care Plan development and review
* Using TPP SystmOne Clinical system
