Job summary Our SET Keyworker service, whichforms part of theNHSLong Term Plan for all Children and Young People with a diagnosis of Autism and/or a Learning Disability with the most complex needs, to have a Keyworker. We havefull timeopportunities as aChildren and Young Person (CYP) Keyworker,whereyou will bebased out of one of our Essex hubs closest to you,with the expectation to travel to families throughout your region. This is a Monday - Saturday service and will requirededicationandflexibility, working between thehours of 8am and 8pmto put children and young people first. We are looking to build a dynamic team who can bring a wealth of experience to ensure that these children, young people and families get the right support at the right time. This is a full time - fixed term position for 12 months. Main duties of the job HCRGCare Group provide a range of children, young people and family services throughout Essex that are free at the point of delivery, which includes ourSET Keyworker Service. Your mainresponsibilities will include but not be limited to: Accept, allocateand assess cases, working alongside the Case Load Manager to identify at-risk children/young people and their families from theDynamic Support Register (DSR) Ensures consistent allocation of cases to build rapport and maintain trusting consistent relationships with CY&P and theirfamilies Leadand participatein the out of hoursrota and providingclinical and caseload supervision. Liaises with Health and social care, Education and Police colleagues to improve access, continuity of care and well planned and well managedtransitions.
