We have an exciting opportunity for a Long Term Conditions Nurse to join our Primary Care team at HMP Bullingdon, a category B prison located in Bicester, Oxfordshire.
HMP Bullingdon is a male remand prison that has a capacity for 1,152 and offers 24 hours healthcare with an inpatient unit on-site.
Hours
Monday - Friday, 37.5 hours
Salary
This position is full time; you’ll receive an annual salary from £46,525 up to £52,507 per annum FTE - depending on experience.

Candidates are required to have a legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
What you'll be doing
As a Long Term Conditions Nurse, your responsibilities will include:
* Ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
* Identifying immediate health needs, including any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing.
* Facilitating arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient.
* Providing proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections.
* Delivering continuity of care across health and social care agencies from courts to custody and community.

Qualifications Required:
* Registered Nurse - General with relevant NMC Registration
* Advanced Nurse Prescriber/ Practitioner
* Educated to degree level
* Knowledge of health and social care models across the criminal justice system and of national and local initiatives relating to physical health
* Experience of assessing the needs of and working with individuals with complex health and social care needs.
