Job summary We are looking for a motivated and dynamic individual to lead the evolving Clinical Nurse Specialist Acute Oncology team. The team works in collaboration with the acute inpatient, Emergency Medicine, Specialised Palliative Care and site-specific Clinical Nurse Specialist teams across STH to ensure that patients presenting acutely unwell with a known or unknown cancer diagnosis or a cancer treatment related problem are managed safely. The Acute Oncology CNS team offer specialist advice and support to the wider multi-disciplinary teams in the management of the complications of treatment or disease for our patients during periods of acute, often complex presentation. Main duties of the job To lead the team to provide safe expert care for patients presenting acutely unwell with a known or unknown cancer diagnosis or a cancer treatment related problem. To work as an autonomous practitioner to accept and prioritise referrals to the service, organising workload, supporting other team members whilst adhering to established clinical protocols and guidelines to assess and diagnose specific AO problems. To provide ongoing support to medical and other health care professionals involved in the patient's management including participating in the AO MDT's presenting patient cases and following up outcomes. To act as a resource in the assessment and recognition of patient's individual physical and psychological needs and offer holistic support to patients, families, carers and colleagues as required. To use relevant specialist training, expertise & knowledge in cancer care and nursing, to provide patients with the necessary information and advice to help them make informed choices. Providing support to patients and their families and referring to other agencies / Health Care Professionals / MDT's as appropriate to optimise the delivery of holistic care. This includes liaison with the patients GP / Primary Care services as required. To attend Malignancy of Unknown Origin/ Cancer of Unknown Primary MDT and clinic to provide support and information for patients; undertaking formal holistic needs assessments at key stages of the patient's pathway.
