Advanced Clinical Practitioner - Inpatients Unit

Based primarily at Paulton Memorial Hospital, with occasional cover at St Martin's Hospital, you will provide advanced clinical care to patients recovering from acute illness, surgery, or requiring specialist rehabilitation. Most patients are transferred from the Royal United Hospital, while others are admitted directly from the community or other secondary care facilities. As an experienced clinician and non-medical prescriber, you will play a vital role in multidisciplinary patient care, working alongside Consultant Geriatricians, specialist doctors, and MDT colleagues to ensure high-quality, patient-centred care.

Main duties of the job
As an Advanced Clinical Practitioner (Nurse or AHP), you will play a crucial role in patient care, clinical leadership, and service improvement. Your responsibilities will include:
* Clinical Practice: Conduct advanced assessments, diagnose, interpret tests, and develop treatment plans. Manage acute patient needs, ensuring timely escalation and response to deterioration.
* Patient Management: Lead clinical care for inpatients, coordinating with multidisciplinary teams (MDT) across primary, secondary, acute, and social care services. Participate in discharge planning and liaise with GPs, carers, and relatives.
* Non-Medical Prescribing: Prescribe and review medications, collaborate with pharmacy teams, and ensure effective medication management throughout a patient's stay.
* Leadership & Supervision: Act as a role model, providing support, professional development, and supervision for colleagues while upholding high standards of care.
* Audit & Research: Lead and participate in clinical audits and research projects to drive service improvements and enhance patient care.
* Education & Training: Support learning and development by mentoring students, delivering training, and fostering a positive learning environment.
* Service Development & Improvement: Identify areas for improvement, contribute to a Just Culture approach, and drive continuous enhancements in clinical practice and patient safety.

The Ideal Candidate
We are looking for an experienced and highly skilled Advanced Clinical Practitioner with a passion for elderly care, frailty, and community services. The ideal candidate will have:
Essential:
* Professional Registration: Registered with the NMC or HCPC, educated to degree level, and a current Non-Medical Prescriber.
* Clinical Expertise: Extensive experience in elderly care, frailty, community services, or a relevant acute care specialty, with highly specialised clinical and technical skills.
* Leadership & Management: Proven experience in leading teams, managing complex caseloads, and working both autonomously and within a multidisciplinary team.
* Quality & Improvement: Experience conducting clinical audits and quality improvement projects.
* Decision-Making & Communication: Strong judgement, analytical skills, and excellent written and verbal communication abilities.
* Flexibility & IT Skills: Ability to adapt to a fast-paced, evolving environment, with good IT skills.
Desirable:
* Advanced Qualifications: A Masters degree and ACP Clinical Badge.
* Other Requirements: A clean driving licence, access to a vehicle for daily use.
