We are currently looking to a recruit an enthusiastic and motivated Recovery Support Worker to join the Solihull Early Intervention Team.
The role will involve offering support to patients in a community and outpatient setting; the applicants must have significant experience of working within a health/social care setting, and it is desirable that you have experience or knowledge of working with Mental Health issues. The post holder will take responsibility for an identified caseload and provide outreach and community follow up to support service users through their recovery using low intensity psychosocial interventions.
You will be working alongside a highly skilled and experienced multi-disciplinary team of Nursing, Psychiatry, Occupational Therapy and Psychology to collaboratively set goals with patients to aid their recovery.
You will ensure that there are effective care pathways to a range of services that will enhance individual recovery. This will be achieved by ensuring close liaison with identified partners and community organisations including employment /training providers, housing and mental health. You will also provide low intensity psychosocial interventions to service users on caseload. You will be able to signpost services users to the appropriate facilities and provide support.
