To work as part of a Neurodevelopmental Nursing Team (NDNT) that provides support and management of children with severe or complex learning disabilities or neuro-developmental disabilities including but not limited to ASD, ADHD, and global-developmental delay.
In conjunction with the operational manager/team leader, to plan, develop, provide and co-ordinate an innovative specialist service for children, young people and their families who are under assessment / need assessing for a potential diagnosis of ADHD via Qb testing, and to provide after care following diagnosis if treatment is required.
To work closely with community paediatricians, parents/Carers, general practitioners, schools and other partners to develop a greater understanding of the conditions and facilitate appropriate support for children, young people and their families.
The post holder will work autonomously and be part of a nursing team that provides a community-based service delivered in schools, patient homes and appropriate clinic and community settings across the Borough.
The post holder will be an experienced nurse with strong clinical leadership skills and the ability to lead and further develop service provision in this specialist area of clinical practice.
Duties and Responsibilities
Clinical
* To act as a source of expertise and demonstrate advanced knowledge skills and experience in the management of complex caseloads including managing own designated caseload and supervising caseload of band 6 nurses and the skill mix team.
* To support less experienced staff in supporting complex cases ensuring they deliver high quality interventions.
* To provide and receive complex information concerning a child/young person/their families and to communicate effectively and sensitively to provide appropriate advice and support as tailored to meet their needs.
* Autonomous practitioner responsible for planning and organising own caseload.
* Accountable for the delivery and development of an evolving specialist nurse led service with ADHD.
* Provide and support a nurse led service that triage, assess, assist in diagnosis, and treat children and young people with neurodevelopment disorders specific to ADHD.
* Work innovatively to address client need.
* To assess, plan and implement packages of care for children & young people, up to 19 years of age, with a diagnosed severe or complex learning disability or neuro-developmental disability (including attention deficit hyperactivity disorder, autism).
* To assess the needs, including challenging or complex behaviour and sleep difficulties, of a child and plan, implement and evaluate individualised care in collaboration with the child, family, and other involved agencies/ professionals.
* To provide advice, support and information to the young person and their family following diagnosis, when needs change or during transition periods; also signposting and/or referring to other sources of support including voluntary and community assets.
* Be empathetic and handle difficult and emotional situations appropriately.
* To provide individual support to assist the child or young person’s development of knowledge and self-management skills in regard to their specific needs and diagnosis relating to ADHD/ASD.
* Contribute to the complex health and social needs assessments of children with a disability and/or emotional or behavioural disorder and offer advice and support to client or other professionals regarding ADHD in this context.
* To promote the health and well-being of children and young people at every contact.
* To act as specialist information, advisory and training resource to health professionals, children’s schools and families and other voluntary and statutory service providers.
* Provide specialist advice around key areas within the aspects of neurodevelopment disorders, specific to ADHD / ASD potentially including sleeping and behaviour management.
* To deliver specialist nurse services in a variety of community settings i.e. child’s own home and the child development centre.
* To have an understanding and knowledge of medications prescribed for ADHD and/or sleep difficulties; supporting medication concordance to improve outcomes for children in accordance with BNF guidance/NICE/Pan Mersey and to hold the V300 non-medical prescribing (NMP) qualification.
* To work autonomously prescribing medication for children and young people including controlled drugs associated with the ADHD condition.
* To develop care pathways in line with NICE guidance as appropriate to the development of the service and in consultation with all key stakeholders.
* To run nurse–led clinics; demonstrating the ability to work as an autonomous practitioner including the recording of the clinical observations, the on-going assessment of treatment effectiveness, monitoring compliance with clinical practice and NICE guidance.
* To offer joint working and specialist advice to other members of the multidisciplinary team and external agencies to implement care plans, packages of care and to evaluate outcomes.
* To liaise and share information with other agencies, with appropriate consent and in accordance to data protection guidelines, in order to support the needs of the child or young person safely and effectively.
* To develop, run and evaluate group support for children, young people with severe or complex learning disability or neuro-developmental disability and their families/carer.
* To ensure that clients are actively involved in all care planning and decision making as per Fraser guidelines and Mental Capacity Act, 2005.
* To contribute to statutory processes such as Education Health & Care Plans as required.
* To use high level communication skills throughout all duties within the clinical role including verbal, non- verbal, and written formats.
* To identify and assess situations and conditions that put a child at risk of abuse/neglect, making appropriate clinical decisions following assessment; following local child protection procedures by seeking support and advice from a peer, manager or named safeguarding nurse as appropriate.
* Attend child protection, core group and family support meetings, participating in the decision-making process. Working in partnership with other agencies monitor and support vulnerable young people.
* Where appropriate, to support families through transition processes from pre-school to education, primary to secondary school and secondary to adult services.
* To discharge from the service appropriately, agreeing a point of closure with the young person, parent or carers and informing relevant professionals.
* Provide a Safe and effective Independent Non-Medical Nurse Led Prescribing Service – Assessing ADHD, reviewing any medical conditions that could specifically impact on assessment / diagnosis or treatment specific to ADHD (as per NICE guidelines, PAN Mersey).
* Providing a safe and effective independent Nurse led prescribing service as an independent prescriber that enables the NMP with titrating and/or changing medication for the children and young people in the service.
* To maintain accurate, comprehensive, and contemporaneous clinical records as an autonomous practitioner.
* Analyse and interpret complex assessment information to develop specialist interventions, ensuring advanced levels of treatment are provided, outcomes are recorded and evaluated to determine clinical effectiveness.
Managerial
* To show a strong commitment to team working, demonstrating respect and consideration to other team members and colleagues in partner agencies.
* To attend and provide leadership in team meetings and confidently represent the service at relevant Trust and external partner meetings.
* To identify any aspects of the service which need to be improved and to advise service and professional managers on appropriate changes.
* In conjunction with team lead and as indicated by service need, to write, develop and implement, review and update policies for the service.
* Represent Trust on strategic and operational groups e.g., carers/ family and subgroups, transitions meeting, children with complex needs group. i.e., DSD, transition, CHC, LD focus groups as delegated.
* As a Specialist Practitioner to act as deputy to the Team Leader when appropriate and offer direction, support, and supervision to the skill mix team.
* To assist in the short-listing and interviewing of multidisciplinary staff as required.
* To provide placements to student nurses as required.
* To receive regular clinical and professional supervision from a more senior clinician according to Trust guidelines.
* Undertake supervisory responsibilities for other team members. Mentor and assess students including post basic students.
* To provide professional clinical and research supervision as well as managerial supervision to qualified nursing staff and students for all aspects of their work.
* To contribute to the development of the knowledge and skills base within the nursing staff and Bridgewater Community Healthcare.
* To effectively manage a caseload, prioritise clinical demands, delegate appropriately to other members of the team and act as a care coordinator for the service.
* Analyse and assess information concerning children and young people with Neurodevelopmental Disorders/ADHD providing specialist expert clinical knowledge to multi-disciplinary groups.
* To be able to identify, manage and review risks undertaking comprehensive risk assessments when necessary.
* To deal with concerns and complaints sensitively, avoiding escalation where possible, in line with Bridgewater Trust policy.
* To learn from incidents, complaints, and user feedback to improve the quality of the service delivery.
* To understand service/team key performance indicators and contribute to the accurate and timely collection of statistical data.
* To take the lead on evaluation of the service and with the use of evidence-based practise, local and national research/audit and patient feedback identify gaps in provision and make recommendations on changes to service provision and clinical working practices to inform operational managers and influence/assist in the commissioning process.
* To lead and facilitate participation of the team in clinical governance initiatives including clinical and record keeping audits and research as part of a process of continuous quality improvement.
* To provide advice, consultation, training and supervision to staff and parents/carers within Bridgewater Community Healthcare Neurodevelopmental Service network across a range of settings including primary care, social services, education, residential units, and family homes.
* To have awareness of the budgets and wider financial pressures to ensure that the clinical team is delivering a cost-effective service.
* To contribute ideas to support the Trust cost improvement plans.
* To be an authorised signatory for approval of annual leave, overtime and car mileage claims and any documentation to acknowledge and confirm that services have been provided to service users by other agencies i.e. interpreters.
* Responsible for ensuring appropriate prescribing of medications, whilst determining the most appropriate cost benefits outcome for Bridgwater Community Healthcare.
* Responsible for the maintenance and monitoring of the safe use of expensive or highly complex equipment e.g. QB Test equipment and ensure that any repairs/replacements are ordered and invoiced appropriately.
Leadership
* To lead and manage quality improvements, encouraging others to recognise the need for change, developing a shared vision, inspiring, and motivating other team members.
* To provide visible nursing leadership to nurses within the service, offering professional and practical advice and support, ensuring standards of nursing are consistently adhered to.
* To provide clinical leadership for staff in the specialist area, including supporting across Bridgewater when required.
* Contribute to the development and implementation of policies, procedures, and guidelines relevant to own area of work.
* To adhere to The Code’s professional standards and behaviour for nurses as required by the Nursing & Midwifery Council (NMC).
* To support, develop and deliver effective supervision systems for nurses in the neurodevelopmental team and across the wider Bridgewater footprint to ensure high quality clinical care and professional development.
* To develop a working knowledge of relevant procedures including safeguarding children, SEN procedures, working with vulnerable adults and other legal frameworks.
* To provide specialist nursing input at senior level across Bridgewater Community Healthcare Neurodevelopmental Services, ensuring effective advice to the Senior Management, Lead Clinicians, other management teams, commissioners, and the wider voluntary and statutory groups.
* To contribute to the workforce development of nurses and skill mix within the team taking a lead role with the operational manager in recruitment and selection processes and training needs analysis to ensure appropriate specialist educational opportunities are available.
* Contribute to the complex health and social needs assessments of children with a disability and/or emotional or behavioural disorder and offer advice and support to client or other professionals regarding ADHD/ASD in this context.
* Attend and where necessary contribute to the assessment of complex health and social needs of looked after children at review meetings.
* Promote health and awareness of Neurodevelopmental Disorders/ADHD through formal and informal education with individuals and groups in a variety of settings.
* To develop care programmes in specialist area to ensure service delivers the highest possible standards of care against national standard and to train and educate staff across the Trust as appropriate.
* To engage in conversations with parents/carers/young people and other professionals to deliver highly specialised advice around specialist subject matter and with particular reference to medication.
Education & Teaching
* To develop, deliver and evaluate tailored awareness training for ADHD, ASD, and other related conditions to members of the multi-disciplinary team and partner agencies.
* To develop, deliver and evaluate group training to young people, parents/carers for ADHD, ASD, and other related conditions.
* Act as mentor/preceptor to new staff and participate, where appropriate, in developing the skills and potential of colleagues.
* To contribute to the development of parent/client information ensuring this is relevant, accessible and up to date in a written and digital format.
Continuing Professional Development
* To ensure as a specialist practitioner/clinical lead you take responsibility for your own personal development/mandatory training in accordance with the trust and NMC/governing body guidelines.
* To participate in individual performance review, ensuring that the objectives set reflect the service and organisational plans.
* To identify personal/professional development within a performance review; to attend relevant training, undertake development activities in order to maintain and enhance skills and knowledge required to maintain up to date registration.
* To demonstrate the ability to reflect on practice with peers, clinical supervisor, manager to identify own strengths and development needs.
* To attend and engage in clinical and safeguarding supervision.
* To use evidence-based practice, maintain skills and knowledge in accordance with current evidence-based research, best practice and Trust polices/guidance.
Physical
* To demonstrate knowledge in moving/handling clients with disabilities.
* To use a range of techniques appropriate for those with severe and profound disabilities including symbols systems, alternative communication equipment and other resources in the provision of support to families.
* Requirement to be physically able to move and transport specialist equipment used within the service across delivery locations within the Borough eg QB test equipment.
