Main duties of the job The LTC Psychological Wellbeing Practitioner will deliver a range of low intensity interventions to clients diagnosed with LTC who have significant social and psychological issues. This will include an assessment of the clients situation with possible follow up sessions of behavioural change therapy, one-to-one guided self help and/or group work informed by the principles of Cognitive Behavioural Therapy (CBT). The post holder will be a qualified psychological wellbeing practitioner, will have a role in mental health promotion and the prevention of mental distress. They will also support the development of psychological knowledge and skills to other primary care practitioners. The post-holder will also liaise with all relevant health and social care partners, present a positive perspective on the mental health issues of people with long term conditions, have a key role in developing projects and initiatives across the area and will also participate in audit and evaluation.
