As a registered nurse, the post holder will provide care to elderly patients with acute health needs in the ward. The post holder will be accountable and responsible for providing a high standard of patient-centred care, carrying out care with direct supervision and support from the team. Effective communication within the multidisciplinary team is essential. The role also contributes towards the effective running of the department, ensuring patient safety and timely care.
Specific responsibilities include:
1. Provision of specialist, high-quality nursing care and support for elderly patients with acute respiratory problems and their relatives/carers.
2. Appropriate delegation of duties to other members of staff for effective delivery of patient care.
3. Providing timely nurse-led interventions for the benefit of patient care.
4. Assessing, planning, implementing, and evaluating care of patients from admission to discharge.
5. Participating in the effective admission, transfer, and discharge of patients involving family, carers, significant others, multidisciplinary teams, and community teams when appropriate.
6. Maintaining effective channels of communication with patients, relatives, and all staff involved in providing care.
7. Providing and receiving nursing handover reports as a team member.
8. Acting as the named nurse for an individual or a group of patients.
9. Being part of the process in implementing clinical governance and understanding the impact this has on patient care, including audit and evidence-based practice.
10. Ensuring that written documentation is maintained in line with the Nursing and Midwifery Council (NMC) standards for Records and Record Keeping and Trust policy.
11. Ensuring that the storage and administration of medicines is carried out according to statutory and Trust regulations.
