An exciting opportunity has arisen for a motivated and compassionate Mental Health Nurse to join our friendly Memory Treatment team.
As an integral part of this role you will be supporting people who have recently received a diagnosis of dementia. You will carry a caseload and provide monitoring of medication, post-diagnostic support, support for carers, signposting to other agencies and supporting people with their carers and families as they adjust to living well with their condition and the changes this brings.
Main duties of the job
As a memory treatment nurse, you will be working in a busy friendly team to provide monitoring of medication, post-diagnostic support and signposting.
You will work autonomously to see people in clinic (Attleborough) and also in their home if they are unable to attend clinic, you will have good assessment and observational skills, a kind and compassionate approach and a true interest in supporting older people.
You will liaise with other providers ie GP, social services, Admiral nurses, Alzheimer's society, carers matters etc to ensure people receive the most appropriate support immediately and in the longer term.
