About the Role
Mid Norfolk Primary Care Network (PCN) has an opportunity for an experienced clinical pharmacist, on a full time basis, to join their growing team, supporting six GP practices across the Dereham area.
The main responsibilities include:
1. Manage the prescription request workflow.
2. Manage complex face to face medication reviews with an emphasis on seeing patients with polypharmacy, deprescribing, and opiate prescribing.
3. Manage network quality activities such as sharing best practice and cross-network audit.
4. Develop and manage medicines optimisation services within the network practices.
5. Provide leadership on quality improvement and clinical audit as well as managing aspects of medicines safety and antibiotic stewardship.
6. Support the management of patients with long-term conditions.
7. Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacist, nurses or GPs on prescribing and monitoring.
8. Work with care home staff to improve safety of medicine ordering and administration.

9. Minimum of 2 years (or equivalent) post qualifying experience as a pharmacist.
10. Completion of an undergraduate degree in pharmacy.
11. Working towards and independent prescribing qualification.
12. Excellent interpersonal, influencing and negotiating skills.
13. Excellent written and verbal communication skills.
14. Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of patients.
15. In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
16. Excellent IT skills.
17. Ability to plan, manage, monitor, advice and review general medicine optimisation issues in core areas for long term conditions.
18. Work effectively independently and as a team member.
Desirable
19. Holding an independent prescribing qualification.
20. Clinical Diploma.
