Job summary The post holder will assist the (Nurse) Manager in delivering a 24-hour Nurse led respite/enhanced support service. They will work in collaboration with the multi-disciplinary team in the assessment, planning, implementation and evaluation of comprehensive care plans and care packages. The post holder will mostly work without direct supervision and they will be responsible for leading and supporting the team in providing good quality care. An adaptable and flexible approach is required due to frequent changing needs of the SU/CYP and service delivery.
