Adult Services provides support to adults over the age of 16 years who have assessed needs. Social Care Worker Practitioners are responsible for providing direct support to individuals and the team in a variety of settings. This can include in an individual's own home or supported tenancy, within a Care Inspectorate registered service (supported vocational activity or short breaks), and in the community, enabling the individual to achieve their goals, identified priorities, and personal outcomes.
