The SW Thames Renal and Transplantation unit is one of the largest renal units in the country and the unit is working towards integrating with St George's University Hospitals NHS foundation Trust to streamline the care pathways of our patient groups.
We are looking for a dynamic and enthusiastic Dietitian to join our team of Renal Dietitians to cover a secondment for 9 months. The post holder will be a responsible for the management of patients on the renal inpatient wards, on dialysis, as well as carrying an outpatient caseload at local renal satellite units. The role is one of an expert practitioner who works autonomously as well as part of a team.
· Take a clinical lead role in the area of weight management and renal dietetics in relation to the kidney transplant pathway.
· To provide specialist dietetic service for renal patients within Sutton and Merton and patients within the catchment area of the South West Thames Renal Unit. This includes providing a renal dietetic service to St. Helier renal inpatients, low clearance clinics, CAPD, and haemodialysis both on site and satellite units and transplantation.
· To be responsible for a designated caseload of patients and provide expert advice to the renal dietetic team.
· To provide expert advice to the screening of renal dietetic referrals.
· To take a lead role in providing learning opportunities and developing resources, for all health care professionals to facilitate the management of renal conditions.
· To provide expert advice to colleagues and other health care professionals on nutritional management of renal patients across the wider community. Also to provide dietetic colleagues and students with learning opportunities to ensure effective use of counselling skills and behaviour change techniques in dietetic interventions, and teaching skills in training.
· To take the lead in an area of clinical practice.
· This role is one of an expert clinical practitioner who works autonomously.
