Care Planning: Develop, implement, and review individualized care plans in collaboration with patients, their families, and other healthcare professionals. Ensure care plans are evidence-based and tailored to meet the unique needs of each patient. Provide education and support to patients and families regarding palliative care options and advanced care planning. Autonomous Practice: Work independently to deliver high-quality palliative care, making clinical decisions within the scope of practice. Utilise advanced clinical skills to manage complex symptoms and provide interventions aimed at improving quality of life. Liaison and Coordination: Act as a liaison between patients, families, primary care providers, specialists, and community services. Coordinate multidisciplinary team meetings to ensure cohesive and integrated care delivery. Establish and maintain effective communication channels with external stakeholders, including hospice services, social workers, and support groups. Documentation and Reporting: Maintain accurate and up-to-date medical records, documenting all patient interactions and care provided. Participate in ongoing education and professional development opportunities. Mentor and support junior staff and students as required.
