Job Title: Complex Care Manager Salary: £45,000 bonuses The role of a Complex Care Manager (CCM) is to organise, facilitate and ensure the delivery of complex home care services, to people with medium to long-term complex care needs living in their own homes. This service is delivered through teams of care assistants and support workers who report to the Complex Care department. Responsibilities The role is field-based, ensuring that high-quality care services are consistently delivered, including covering emergency shifts when needed. Maintain regular communication with service users, ensuring that Person-Centered Care principles are upheld. Client Liaison: Regularly communicate with clients to understand their preferences for contact with the Complex Care Manager (CCM). Discuss care standards, address any concerns, and ensure their needs are being met with the highest level of care. Collaboration with Clinical Team: Work closely with clinicians to regularly review care packages, ensuring they align with established clinical standards. Take proactive steps to address any discrepancies or issues, implementing timely corrective actions as necessary. Reporting: Prepare and submit monthly reports to the Clinical Commissioning Group (CCG), detailing the ongoing care provided, ensuring transparency and compliance with service expectations.
